Polythene cover on manway possibly causes fire

After the days jobs of partially unloading catalyst from a reactor was completed, contractors observed the weather was cloudy and covered the manhole opening with a polythene cover. Some time later a flash fire occurred inside the reactor, possibly due to residual vapours from remaining catalyst ignited by the static electricity generation from the polythene cover.

Enforcement of rules in PSM

The human fears only when rules are enforced. In Singapore, I saw this sign on a pedestrian foot overbridge. Though I have used this overbridge more than 20 times, I never saw anyone breaking the law. Please observe that the sign says that the fine is 1000 Singapore dollars.

What does this have to do with process safety management? Many companies do have a carrot and stick policy towards PSM but I have also noticed that the stick is not used properly and effectively many times. Unless employees (from the top to the bottom) observe discipline in manufacturing, accidents in process industries will continue to occur!

Dangerous dead legs

An incident of a hydrocarbon leak in a plant occurred due to a dead leg. An oil leak was traced to a small hole at the lower end of a pipework elbow. The line was not in use and had the stagnant liquid created a deadleg which caused the corrosion. The CSB video "Fire from ice" depicts the hazards of dead legs. Are you looking at dead legs?
