An experienced engineer went inside a confined space in a ship. Unfortunately, the latches on the entrance hatch closed by gravity once he was inside and he could not come out. He was found dead later. Read the incident in this link

Scarry.. similar incident happened in my plant where an operator who got trapped inside a stripper was locked from out side by mechanical technician has survived by knocking the shell with metal rod in rythmic manner. Before starting luckily one of the operating team member has noticed this sound and decided to get it opened and checked.
